About the PositionWe are seeking a highly motivated and organized Outpatient Care Coordinator to join our team. As an Outpatient Care Coordinator, you will be responsible for managing patient care and facilitating transitions between levels of care. This role requires excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
Key Responsibilities- Manage patient care and facilitate transitions between levels of care
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and compliance with care plans
- Collaborate with interdisciplinary teams to coordinate patient care and services
- Graduate from an accredited Registered Nursing or Vocational Nursing Program
- Current CA RN license or CA LVN license, current CPR certification, valid CA Driver's license
- 3 years acute care or case management experience preferred
- 2-3 years of utilization or HMO experience preferred

The estimated salary for this position is $45.00 - 50.00 per hour, depending on experience and qualifications. We offer a competitive compensation package, including a sign-on bonus and discretionary awards, in addition to a full range of medical, financial, and other benefits.
