Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare in Erie, PA. As an LPN, you will have the opportunity to work in post-acute facilities, providing high-quality care to residents while enjoying a flexible schedule and competitive pay.

Responsibilities
  • Provide direct patient care, including administering medications, monitoring vital signs, and conducting intakes
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved LPN educational program
  • High school diploma or equivalent
  • Minimum of 6 months of experience as an LPN or in a related role
