Job Summary

The Bridge Inc. is seeking a highly skilled Care Coordination Supervisor to join our team. As a key member of our Health Home program, you will be responsible for overseeing the daily activities of our Care Coordination team and ensuring the efficient and effective delivery of care management services.

Key Responsibilities
  • Supervise and oversee the daily activities of the Care Coordination team.
  • Utilize electronic databases as required by Health Homes requirements.
  • Participate in group and individual supervision to ensure that services for Health Home consumers are being rendered appropriately.
  • Initiate ongoing team meetings with Care Coordinators to implement and review goals, plans of care, comprehensive assessment, HARP, core services, and timely documentation.
  • Service caseloads of specific individuals needing additional expertise and stabilization, including AOT consumers who will be stepped down to Care Coordination.
  • Address issues and concerns that may arise for clients throughout the enrollment period within the Health Home program.
  • Provide ongoing monitoring to ensure that client charts and documentation are in compliance with City, State, and Federal guidelines.
  • Provide direct one-to-one and group clinical supervision for the Care Coordinators.
  • Provide support around case conferencing and coordinate incident reporting procedures for the program.
  • Complete all weekly and monthly reports as required.
Requirements
  • Master's Degree Required
  • 2 years of supervisory experience
  • Excellent computer skills and knowledge of AWARDS
  • Good personnel management skills to work in a human services/rehabilitation environment
  • Excellent verbal and written communication skills, professional presentation, and excellent interpersonal/customer service skills
  • Bilingual is highly desirable
