Responsibilities:
  • Conduct clinical case reviews and provide clear communication with patients, clinicians, and facility leaders regarding care and services for enrollees.
  • Work alongside pharmacists on medication management and implement an individualized plan of care for patients' goals.
  • Make recommendations on transitional care, DME, home health services, or outpatient services after short stay.
  • Timely response in chart completions and pharmacy recommendations.

Requirements:
  • Experience as a clinician in a skilled nursing facility preferred.
  • Physicians trained or practicing medicine in family (FM), internal (IM), emergency (EM) geriatric, primary, adult, post-acute, home health, hospice, and palliative medicine encouraged to apply.
