Key Responsibilities- Assess and Plan Care: Conduct thorough assessments of patients' physical, social, and environmental needs, and develop individualized care plans to address these needs.
- Provide Direct Patient Care: Deliver high-quality, patient-centered care to patients in their homes, including administering medications, wound care, and other treatments as needed.
- Collaborate with Interdisciplinary Team: Work closely with physicians, social workers, and other healthcare professionals to ensure seamless care coordination and communication.
- Document Patient Information: Accurately and thoroughly document patient information, including assessments, care plans, and progress notes.
- Communicate with Patients and Families: Provide clear, compassionate communication to patients and their families, addressing their concerns and needs in a timely and respectful manner.
- Participate in Quality Improvement: Contribute to quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care and outcomes.
- Education: Associate's Degree in Nursing
- Experience: 3 years of experience in clinical nursing, preferably in home health or hospice care
- Licensure: Registered Nurse license issued by the state in which the team member practices
- Certifications: Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA)
- Experience with Electronic Medical Records (EMRs): Familiarity with electronic medical records systems and ability to navigate complex software
- Strong Communication Skills: Excellent verbal and written communication skills, with ability to communicate effectively with patients, families, and healthcare professionals
- Problem-Solving Skills: Ability to analyze complex patient situations and develop effective solutions
