Position Overview:
The Care Coordinator is essential in assisting the Care Manager by increasing billable hours, ensuring compliance with regulations, providing exceptional support to seniors and individuals with disabilities, and creating a positive work environment for our caregiving team.
Key Responsibilities:
Client Evaluations:
- Conduct meetings with clients and their families in their respective care environments.
- Collect and document assessment data using established protocols.
- Gather relevant information from various stakeholders, including insurance representatives and financial advisors.
- Ensure assessments are thorough, precise, and based on comprehensive data.
- Identify risk factors, obstacles to achieving goals, and strengths that can aid clients.
- Perform supervisory visits with clients every 90 days.
Care Planning:
- Develop detailed care plans that address all relevant areas identified in assessments.
- Establish measurable, time-sensitive, and achievable goals with clients and their decision-makers.
- Integrate private and community resources into care plans.
- Assist in coordinating client care with external resources, attending appointments as necessary.
- Ensure all care plans are created and updated promptly.
Monitoring and Evaluation:
- Participate in quality management initiatives.
- Document progress and conduct evaluations with clients and their families.
- Ensure timely updates to care plans based on reassessments or changing client needs.
- Communicate any modifications to care plans to caregivers and the operations team without delay.
Caregiver Training and Management:
- Conduct orientations for new caregivers and provide ongoing training.
- Ensure caregivers are trained according to company standards and maintain the necessary certifications.
- Perform supervisory visits for caregivers and facilitate continued training.
- Address conflicts within the caregiving team and collaborate with the Care Manager to allocate resources effectively.
Regulatory Compliance:
- Ensure adherence to local licensing laws and Medicaid regulations.
- Prepare for state surveys and maintain readiness.
- Participate in quarterly performance improvement meetings.
Communication:
- Assist with incoming company calls.
- Help prepare agendas and participate in team meetings.
- Serve as a positive role model in interactions with clients, staff, and the public.
Administrative Duties:
- Create weekly and monthly schedules for assessments and orientations.
- Provide support for the Care Manager and conduct client intake assessments as needed.
- Assist with scheduling and recruitment support when necessary.
- Maintain updates on client status and document all relevant communications.
- Perform other related tasks as assigned.
Qualifications:
- Exceptional customer service skills.
- Experience in caregiving, with proficiency in advanced transfer techniques and hands-on care.
- Proficient in using technology and computer systems.
- Strong organizational abilities and attention to detail.
- Team-oriented with flexibility in the workplace.
- Strong analytical and problem-solving capabilities.
- Ability to thrive in a fast-paced and sometimes high-pressure environment.
- Reliable transportation and a clean driving record.
Education and Experience:
- Minimum of one year in care supervision, coordination, or management.
- Ability to achieve certification as a DCW Trainer.
Physical Requirements:
- Prolonged periods of sitting and working on a computer.
- Must be able to travel within the designated service area.
Additional Requirements:
- Must be a Notary or obtain Notary certification.
