  • Conduct medically necessary utilization reviews, manage home/outpatient services, and provide discharge planning coordination and intervention.">
  • Communicate effectively with leadership and stakeholders to ensure timely updates and data-driven decision-making.">
  • Demonstrate Service Excellence at all times and perform other duties as assigned.">

Requirements

To be successful in this role, you will need:

  • ">
  • A minimum of 2 years of experience in case management or a related field.">
  • A Bachelor's Degree in Nursing or a related field.">
  • A valid State of Florida Registered Nurse (RN) License.">
  • Basic Life Support (BLS) Certification.">
  • Case Management Certification (CMC).">
  • Experience with InterQual and other electronic medical record systems.">
