Job Summary

This position is responsible for providing casework services to participants, including placement assessment, case planning, and case management. The Case Manager will work closely with the Department of Adult and Juvenile Justice to ensure that participants receive the necessary support and resources to achieve their goals.

Key Responsibilities

* Provides interactive services to participants, including counseling and guidance
* Conducts placement assessments and develops case management plans
* Develops and monitors participant progress relative to their case management plan
* Makes referrals to outside agencies as necessary
* Maintains progress notes in participant files

Requirements

* High school diploma or equivalent and a minimum of three years of experience working with criminal justice populations
* Bachelor's degree in human services or a related field preferred
* Valid driver's license required
* Possession of an Alcohol and Drug Counselor Certification recognized by the Department of Health Care Services (DHCS) preferred
