Job Summary

We are seeking a highly motivated and detail-oriented Case Management Trainee to join our team at St. Elizabeth's Medical Center. As a Case Management Trainee, you will be responsible for coordinating services and supports for individuals with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act.

Key Responsibilities
  • Coordinate consumer assessment, including medical, psychological, educational, and vocational evaluations
  • Develop and implement individualized program plans (IPPs) to meet the unique needs of each consumer
  • Provide advocacy services to consumers and their families, including attending IEP and ITP meetings
  • Complete necessary paperwork, reports, and documentation on a timely basis
  • Collaborate with other programs and agencies to ensure seamless service delivery
Requirements
  • Current enrollment in a Bachelor's degree program in social work, psychology, or a related field
  • Ability to work cooperatively and effectively with others
  • Strong communication and interpersonal skills
  • Ability to handle change well and be flexible and adaptable in dealing with interruptions, new priorities, and new assignments
