The Team Lead Community Health Advocate will provide front-line leadership to the Community Health Advocate team at the assigned facility, performing the responsibilities of a Community Health Advocate.

Key Responsibilities:
Advocate for individual and community health equity
Assist patients in accessing health-related services, including obtaining a medical home, providing instruction on appropriate use, and overcoming barriers to obtaining needed medical care and/or social services
Assist the Program Coordinator in monitoring performance issues and contribute to the performance evaluation process
Monitor and arrange for adequate staffing to ensure appropriate Community Health Advocate coverage for the hospital
Collect, track, and report information about the community and community benefits
Document all patient encounters, complete and submit monthly reports, and maintain comprehensive electronic client files

Requirements:
High School Diploma or GED
2 years' experience as a community health advocate
Certified Community Health Worker in the State of Maryland
Participation with a community organization, verified by personal or professional reference with personal knowledge of this fact
Residency within the target population
