Key Responsibilities:

  1. Deliver skilled physical therapy services aligned with IEP and IFSP requirements, as well as physician directives.
  2. Collaborate with families, educators, and other professionals to ensure effective treatment plan implementation.
  3. Administer therapies aimed at developing and restoring patient functionality, employing appropriate modalities and techniques as per the established care plan.
  4. Monitor and document patient responses to treatments, promptly reporting any changes in condition to the relevant healthcare team members.
  5. Educate patients, families, and other healthcare personnel about the physical therapy process and their roles in supporting the patient’s recovery.
  6. Conduct ongoing assessments of patient rehabilitation needs, modifying care plans as necessary to reflect changes in patient status.
  7. Ensure that care plans comprehensively address all identified issues, providing justifications for any omissions.
  8. Develop measurable patient outcomes and discharge plans tailored to individual needs.
  9. Submit timely documentation, including progress notes and evaluations, in accordance with service mandates and insurance requirements.
  10. Adjust treatment plans based on patient progress and obtain necessary approvals for modifications.
  11. Evaluate service effectiveness through regular monitoring, ensuring at least quarterly assessments.
  12. Guide and instruct patients and caregivers in activities that support treatment goals.
  13. Recommend necessary materials or equipment to enhance treatment efficacy.
  14. Participate in interdisciplinary meetings for case reviews and program adjustments.
  15. Initiate and execute diagnostic and rehabilitative procedures, ensuring patient safety and care continuity.
  16. Maintain accurate and timely patient care records, submitting evaluation reports within specified timeframes.
  17. Engage in professional development through in-service training and quality improvement initiatives.

Job Types: Full-time, Part-time

Compensation: $85.00 per hour
