Nurse Care Coordinator

Job Summary:
We are seeking a skilled Nurse Care Coordinator to join our team at RWJBarnabas Health. As a Nurse Care Coordinator, you will play a critical role in providing comprehensive care coordination for patients, ensuring seamless transitions between healthcare settings and optimizing patient outcomes.
Key Responsibilities:
- Evaluate medical necessity and appropriateness of care, optimizing patient outcomes
- Facilitate patient throughput to a final destination facility with a focus on quality and efficiency
- Coordinate patient placement among multiple outreach and network facilities for admissions and transfers
- Manage individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes
- Establish and promote collaborative relationships with physicians, payers, and other members of the healthcare team
- Collect and communicate pertinent, timely information to payers and others to fulfill utilization and regulatory requirements
Requirements:
- Active BSN RN license in state of practice or compact RN licensure
- Critical Care Experience (ALS/PALS/BLS) with three years of experience in care of the population served
- Working knowledge of hospital operations, medical/nursing staff procedures, hospital and community resources
- Critical thinking, conflict resolution, time management, and problem-solving skills
- Excellent communication and organizational skills
Preferred Qualifications:
- Expert level of Microsoft Office
- Bachelor's degree in nursing or healthcare-related field
- Additional related education and/or experience
