Responsibilities:
  • Care Coordination: Collaborate with multidisciplinary teams to develop and implement comprehensive care plans, ensuring patients receive the right care, at the right time, by the right team.
  • Relationship Building: Foster strong relationships with post-acute care facilities, hospitals, and community-based providers to facilitate effective care transitions and optimize patient outcomes.
  • Discharge Planning: Work closely with patients, families, and healthcare providers to ensure timely and safe discharges, reducing readmissions and improving patient satisfaction.
  • Population Health Management: Contribute to the development and implementation of population health strategies, focusing on high-risk patients and those with complex needs.
  • Quality Improvement: Participate in quality improvement initiatives, analyzing data and implementing evidence-based practices to enhance patient care and outcomes.
Requirements:
  • Education: Associate's degree in Nursing from an accredited institution.
