Transitioning Patient Care Professional

We are seeking a skilled Transitioning Patient Care Professional to join our team at TotalMed. As a key member of our interdisciplinary team, you will provide specialized nursing care to patients transitioning from intensive care to general medical-surgical units.
The ideal candidate will possess strong communication and problem-solving skills, with at least two years of recent Step Down experience. We offer a salary of $1,931.77 per week, plus comprehensive benefits and opportunities for professional growth.
About the Role:
- Monitor and assess patients' vital signs, cardiac rhythms, and respiratory status in the step-down unit.
- Administer medications, perform procedures, and collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Educate patients and families on post-intensive care recovery, medication management, and self-care techniques.
