Job Title: Diversion Case Manager

We are seeking a highly skilled and compassionate Diversion Case Manager to join our team at Community Teamwork. As a key member of our homeless services team, you will play a critical role in providing case management services to clients and their families, helping them to overcome barriers and achieve self-sufficiency.

Responsibilities:
  • Advocate on behalf of program clients to help them move forward out of homelessness.
  • Work with clients to develop short-term plans to obtain stable housing, treatment options, employment strategies, and/or to move toward economic self-sufficiency.
  • Complete intake and assessment to identify guests' strengths, needs, barriers, and goals in order to achieve shelter diversion or rapid rehousing.
  • Develop and implement individual action plans within 48 hours of assessment.
  • Work with outside providers to educate the community about shelter services and appropriate referrals.
  • Help guests apply and complete applications for benefits.
  • Develop and maintain positive working relationships with local landlords, property managers, housing authorities, social service organizations, state agencies, and internal partners.
Requirements:
  • High School Diploma or Equivalent.
  • Prior related experience preferred.
  • Assessment, advocacy, and case management work in a Human Service field preferred.
  • Knowledge and experience working with low-income, culturally diverse, and at-risk families and individuals.
  • Demonstrated knowledge of community resources available to low-income individuals and families.
  • Excellent customer service, organizational, oral, and written communication skills.
  • Ability to manage multiple priorities and work in a fast-paced environment.
  • Proficient computer skills required in basic computer software and data collection systems.
  • Excellent crisis management skills.
  • Ability to maintain accurate records and confidentiality of information.
