Position Summary: The Care Manager is responsible for delivering supportive services to clients, ensuring their dignity and privacy are upheld. Services may encompass companionship, concierge assistance, medication management, personal care, transportation, and any other needs outlined in the client's service plan.

Qualifications and Requirements:

  • High School diploma or equivalent preferred.
  • Ability to read, write, and follow instructions.
  • Must be at least 18 years old and demonstrate competency.
  • If under 18, must be a high school graduate or enrolled in a vocational program with demonstrated competency.
  • Reliable transportation is necessary.
  • Ability to work positively with clients, families, and team members.
  • Exhibit compassion, responsibility, and a positive demeanor.
  • Must not be listed on the Nurse Aide Registry or Employee Misconduct Registry for any findings related to client abuse, neglect, or misappropriation of property.

Key Responsibilities: The individual in this role must be capable of performing the following essential functions, with or without reasonable accommodations:

  • Complete scheduled visits/tasks, which may involve multiple clients across various locations.
  • Attend mandatory orientation, in-service training, and staff meetings.
  • Adhere to all agency policies and procedures, including documentation standards.
  • Maintain confidentiality of client information.
  • Provide services as specified in the service plan, including companionship, concierge assistance, medication management, personal care, or transportation.
  • Identify and report any safety hazards in the client's environment, changes in client condition, or any complaints to the supervisor.
