We are seeking a highly skilled and compassionate Registered Nurse Coordinator to join our Oncology Multispecialty Care Centers team. As a key member of our Disease Team, you will play a vital role in navigating patients and families across care settings, ensuring seamless coordination of care, and providing exceptional patient-centered care.
Key Responsibilities- Collect and analyze patient data to assess current and projected needs, developing a comprehensive patient education plan to address teaching and learning topics.
- Collaborate with Disease Team members to ensure coordination of care across the continuum, facilitating patient participation in treatment planning sessions and documenting treatment plans.
- Identify candidates for open clinical trials and communicate with physicians, mid-level providers, and research nurses to ensure follow-through on clinical trials accrual.
- Oversee Intake Assistant performance, analyzing accuracy and completeness of data compiled on new patients and providing feedback to supervisors.
- Set and strive to meet annual goals for professional development, staying up-to-date with the latest clinical research and best practices.
- BSN required, master's preferred.
- 3 years oncology or surgical nursing experience.
- Connecticut RN license.
- Oncology Nursing Society certification (OCN, AOCN) desirable.
- Ambulatory experience a plus.
- Epic and Microsoft office experience strongly preferred.
