8:00 AM - 5:00 PM

Key Responsibilities

As a Pharmacist, your duties will include:

  • Assessing and overseeing medication therapy in relation to the comprehensive needs of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
  • Complying with organizational policies and procedures as well as all relevant state and federal pharmacy regulations.
  • Adhering to accreditation standards and engaging in quality assurance initiatives.
  • Facilitating new patient referrals and admissions through teamwork with nursing staff, customer service, and distribution channels.
  • Reviewing, receiving, and validating all new patient orders, documenting them accurately.
  • Assisting healthcare professionals with:
  • Planning and executing patient care in home settings;
  • Establishing therapeutic goals for medication therapy;
  • Setting criteria for monitoring the safety and effectiveness of drug therapies;
  • Preparing regular updates on patient progress.
  • Conducting and documenting initial and ongoing assessments of specific patient, medication, and disease information to identify actual and potential medication-related issues.
  • Monitoring medication therapy outcomes by collecting, analyzing, and interpreting laboratory and clinical data along with other relevant patient information.
  • Providing consultations and addressing inquiries from patients, families, physicians, and other healthcare professionals regarding medications and their applications.
  • Maintaining thorough and appropriate documentation of all clinical activities and communications in the patient record.
  • Processing and verifying all medications, supplies, and equipment prior to dispensing.
  • Keeping clinical skills and pharmacotherapy knowledge current by reviewing literature and participating in educational seminars and conferences.

Qualifications

  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
  • Registered Pharmacist License in applicable states.
  • A minimum of two (2) years of recent experience in an acute-care environment or one (1) year of recent experience in home infusion therapy.
  • Experience in compounding infused medications.
  • Demonstrated ability to evaluate and monitor medication therapies for home care patients.
  • Strong organizational and time-management capabilities.
  • Active participation in professional organizations is preferred.
  • Excellent verbal and written communication skills.
  • Willingness to work flexible hours, including on-call evenings and weekends as scheduled.
