Position Overview
As a Clinical Pharmacist, your role will encompass the evaluation and management of drug therapies tailored to the comprehensive needs of home care patients, collaborating closely with physicians, nurses, and other healthcare professionals.
Work Schedule
Shifts will vary from Monday to Friday, typically from 8:00 AM to 5:00 PM, with weekends required as necessary.

Key Responsibilities
As a Clinical Pharmacist, you will be responsible for:
- Assessing and monitoring drug therapy in relation to the overall needs of home care patients.
- Ensuring compliance with organizational policies and all relevant state and federal pharmacy regulations.
- Participating in quality assurance initiatives and adhering to accreditation standards.
- Facilitating new patient referrals and admissions through effective collaboration.
- Reviewing and verifying all new patient orders, documenting appropriately.
- Assisting healthcare professionals in planning and implementing patient care, setting therapeutic goals, and preparing progress reports.
- Conducting thorough assessments to identify drug-related issues and monitoring drug therapy outcomes.
- Providing expert consultation and addressing inquiries from patients and healthcare providers regarding medications.
- Maintaining accurate documentation of all clinical activities and communications.
- Processing and verifying medications and supplies prior to dispensing.
- Keeping abreast of pharmacotherapy advancements through continuous education.
Qualifications
To be considered for this role, candidates should possess:
- A Bachelor of Science in Pharmacy; a Doctor of Pharmacy is preferred.
- A valid Registered Pharmacist License in the relevant state(s).
- A minimum of two years of recent experience in an acute-care environment or one year in home infusion therapy.
- Experience in compounding infused medications.
- Strong organizational and time-management abilities.
- Membership in professional organizations is advantageous.
- Excellent verbal and written communication skills.
- Flexibility to work on-call during evenings and weekends as required.
