Wellbe Senior Medical is seeking a highly skilled and compassionate Physician Assistant - Geriatric Care Specialist to join our interdisciplinary team. As a key member of our care delivery team, you will be responsible for providing high-quality, patient-centered care to our geriatric patients in their homes.
Responsibilities
- Conduct in-home assessments and develop comprehensive care plans for patients with complex medical needs.
- Provide primary care services, including medication management, wound care, and palliative care.
- Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to ensure seamless care coordination.
- Engage patients and their families in the care planning process, promoting patient empowerment and self-management.
- Document patient interactions and care plans in our electronic health record system.
- Participate in regular team meetings and contribute to quality improvement initiatives.
Requirements
- Master's degree in Nurse Practitioner studies from an accredited program.
- Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the state of practice.
- Current board certification as a Nurse Practitioner.
- Experience in geriatric care, home health, or palliative care preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
