About the Role

The Estates of Perryville is seeking an experienced Registered Nurse (RN) to fill the position of MDS Coordinator. As a key member of our team, the MDS Coordinator will play a crucial role in ensuring the highest quality of care for our patients.

Key Responsibilities
  • Manage MDS assessments to evaluate patient health, quality of care, and well-being
  • Ensure accurate and consistent completion of MDS paperwork, as required for Medicare and Medicaid funding
  • Assess patient care and complete forms for each patient, involved in the admissions process, and regularly update assessments
  • Collaborate with the healthcare team to ensure precision in completing assessments and accurate data entry
Requirements
  • Valid Registered Nurse (RN) license in MO
  • 1 year of MDS/care planning experience
  • Experience in long-term care preferred
