Position Overview:
We are pleased to announce an opportunity for a Clinical Coordinator in Cardiology. This role is essential in managing the holistic care of patients experiencing heart failure, guiding them from their initial consultation through to their post-discharge recovery. The selected candidate will be responsible for assessing and planning tailored care during hospitalization, coordinating discharge strategies, educating patients and their families, and advocating for patients throughout their healthcare journey.

Key Responsibilities:

  1. Oversee the clinical management of heart failure patients throughout their care journey, from admission to follow-up.
  2. Engage effectively with patients, families, and caregivers to ensure clarity and adherence to care plans.
  3. Provide in-depth education to patients, families, and nursing personnel regarding heart failure treatment and self-management techniques.
  4. Administer daily oversight of the heart failure program, including resource management, scheduling, and documentation processes.
  5. Lead and evaluate care protocols, monitor quality outcomes, and identify areas for process enhancement.
  6. Facilitate the coordination of discharge medications, including assisting with prior authorizations to ensure seamless care continuity.
  7. Conduct follow-up calls post-discharge to evaluate patient progress, address any concerns, and arrange necessary follow-up appointments.
  8. Assist with genetic counseling related to heart failure as needed.

Qualifications:
To be considered for this role, candidates must possess a Bachelor's Degree, with a Master's Degree preferred. A minimum of four to five years of recent full-time nursing experience is required, with teaching experience being advantageous. Candidates should have two to three years of recent experience in a relevant specialty, such as Cardiology. A valid and current Professional Nurse license in NYS is mandatory.

Preferred Qualifications:
In addition to the minimum qualifications, candidates with experience in program development, data analysis, and a strategic mindset similar to that of a marketing manager will be favored. Demonstrated ability to manage in-house operations, enhance patient experiences, monitor performance metrics, and communicate effectively with outpatient and referring physicians is essential. A commitment to delivering high-quality patient care and engaging in continuous improvement initiatives is expected.
