We are seeking a skilled Palliative Care Home Visiting Clinician to join our team at Northwestern Medicine. As a Palliative Care Nurse Practitioner or Physician Assistant, you will provide high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Provide comprehensive palliative care services to patients in their homes
- Conduct thorough health assessments and develop individualized care plans
- Prescribe and manage medications, therapies, and other treatments as needed
- Collaborate with healthcare teams to ensure seamless care coordination
- Provide education and support to patients and their families
- Masters degree from an accredited school of nursing or physician assistant program
- Current license as a Registered Nurse in the State of Illinois and Advanced Practice Registered Nurse certification
- Privileged and credentialed through the Northwestern Medicine Medical Staff Office
- Cardio-Pulmonary Resuscitation BLS certification
- Palliative care, hospice, ED, or ICU experience
