About the Role

At Horizon Home Care & Hospice, we're dedicated to providing exceptional care to our patients in the comfort of their own homes. As a Home Health Aide, you'll play a vital role in our interdisciplinary Hospice team, delivering personalized care and support to patients and their families.

Key Responsibilities
  • Provide direct assistance with activities of personal care, maintenance of patient health, and facilitation of treatment under the direction of a Registered Nurse or Therapist.
  • Support patient care by reviewing care plans, notifying Case Managers of issues or concerns, and revising care plans as needed.
  • Assist with daily living tasks, such as bathing, dressing, transfers, and toileting, under the supervision of the Case Manager.
  • Explain care being provided to patients and involve them in their care.
  • Complete mobile care upon arrival and departure from patients' homes, including tasks completed in accordance with the care plan, and maintain accurate mileage logs.
  • Work cooperatively with patients, care providers, and family members, as well as with Agency staff, by listening and responding to patient needs, engaging in appropriate communication, and reporting pertinent patient information to Case Managers in a timely manner.
  • Regularly complete home maintenance tasks, such as house cleaning, washing dishes, laundry, meal preparation, and transportation, as outlined in the plan of care.
