Job Title: CNA-Home Health Aide

Horizon Home Care & Hospice is seeking a compassionate and dedicated Home Health Aide to join our team.

About the Role:

As a Home Health Aide, you will provide personalized care and support to patients in the comfort of their own homes. You will work closely with our interdisciplinary team to deliver high-quality care and improve patient outcomes.

Responsibilities:
  • Provide direct assistance with activities of daily living, such as bathing, dressing, and toileting.
  • Support patients with mobility issues, including transfers and range of motion exercises.
  • Assist with meal preparation, light housekeeping, and laundry.
  • Monitor patients' vital signs and report any changes to the Case Manager.
  • Work cooperatively with patients, families, and healthcare providers to ensure seamless care.
