Job Responsibilities- Provide primary care services to a diverse patient population, including families and individuals with a range of health concerns.
- Work collaboratively with a medical assistant, behavioral health clinicians, and community health workers to deliver comprehensive care.
- Manage a panel of patients, with a focus on preventive care, acute illness, and chronic disease management.
- Utilize EPIC EMR system to document patient information and communicate with colleagues.
- Participate in scheduled shifts, including 4 days of patient care and 1 administrative day per week.
- Active Licensed PA or NP in the state of Oregon.
- Current DEA License.
- 2 or more years of experience, preferably in rural community medicine.
