Valeo Resources is partnering with a non-profit community health organization to fill a full-time NP/PA position in San Francisco, CA.

About the Role

The Integrated Care Center Clinic is a federally qualified health center providing primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We serve patients in need, including those experiencing homelessness, mental illness, substance use, and chronic medical conditions.

Responsibilities
  • Provide comprehensive care to patients in a team-based delivery model.
  • Document care promptly and follow charting protocols and applicable CA state licensing requirements.
  • Complete patient sessions in a timely manner.
  • Ensure follow-up for all aspects of ordered care, including specialty referrals, laboratory results, and radiology reports.
  • Respond to patient requests for information and assistance in a timely manner.
  • May supervise students as appropriate.
  • Maintain availability for administrative and clinical duties as requested by supervisor.
Requirements
  • Graduation from an accredited school of nursing with a master's degree.
  • A minimum of 2 years post-graduate experience working in a clinical setting as an NP/PA.
  • Possession of a valid California license as an ANP or FNP with furnishing number.
  • Possession of national certification from the AANP.
  • Possession of BCLS/CPR certification.
  • Possession of DEA Controlled Substances Registration Certificate.
Desired Qualifications
  • Bilingual Spanish/English.
  • Experience with electronic medical records.
  • LGBT Cultural competence and ability to work with a diverse population.
  • Comfort working on teams and with safety net populations.
