About the Role

This is an exciting opportunity to work in a dynamic and supportive environment where you will have the chance to develop your skills and expertise as a Licensed Practical Nurse. You will work closely with our experienced healthcare team to provide exceptional patient care, support, and education.

Responsibilities
  • Provide high-quality patient care and support to patients with end-stage renal disease or chronic kidney disease
  • Work collaboratively with our healthcare team to develop and implement individualized patient care plans
  • Conduct patient assessments, administer medications, and provide education on dialysis treatment and self-care
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Requirements
  • Current LPN-LVN license in the state of practice required
  • Current CPR certification required
  • IV certification required if required by state regulations
  • High school diploma or equivalent required
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
  • BONENT or CHT certification preferred or as required by state regulation
  • Minimum of one year's experience in a hospital or clinical setting preferred
