Job Description

Job Title: Director of Resident Care and Service Coordination

Reports to: Executive Director

Purpose: The Director of Resident Care and Service Coordination is responsible for the overall health care management of the residence, overseeing the Residents' activities of daily living. This includes safety standards, quality assurance, infection control, admissions, service referrals, and special projects.

Responsibilities:

1. Resident Care:

• Conduct daily rounds to check on resident and unit cleanliness

• Implement services for resident move-in, helping residents with orientation to the assisted living community and staff

• Inform emergency contacts and staff of changes in a resident's condition and needs

• Ensure all medications are available and residents receive their medications as ordered

• Counsel residents and their family members in meeting residency standards and health care/psychosocial needs

2. Admissions:

• Serve as the primary contact for new admission inquiries

• Complete pre-admission assessments and determine financial eligibility in collaboration with the Special Care Residence Manager

• Assist the Executive Director in implementing marketing plans, including networking with potential referral sources

3. Case Management:

• Ensure effective case management and follow-up practices

• Assist residents and families with financial, social security, Medicare, and insurance issues

• Participate in multidisciplinary care plan meetings, identifying problems, setting goals, and reviewing outcomes

4. Employee Management:

• Supervise the performance of licensed and non-licensed nursing staff

• Initiate the disciplinary process as outlined in the Rubin Home Assisted Living employee handbook

Requirements:

• Associate's degree or higher in a relevant field, such as human service management, housing management, social work, or nursing

• Massachusetts driver's license with a safe driving history

• At least 5 years of experience in community health, human services, or assisted living

• Excellent organizational, communication, and social skills

• Ability to work in a team and develop and maintain facility census

• Strong understanding of assisted living, Medicaid, Medicare, and SWH programs

• Ability to deal tactfully with all residents, family members, visitors, government agencies/personnel, and the general public

Physical Requirements:

• Frequent bending for filing

• Ability to lift up to lbs.

• Ability to sit at a desk for extended periods and walk throughout the facility

• Annual physical exam and negative Mantoux test (or chest X-ray)

• Ability to tolerate frequent stair climbing

Working Conditions:

• Frequent exposure to residents with varying health conditions

• Ability to work in a fast-paced environment with multiple priorities

• Must be able to maintain confidentiality and handle sensitive information
