Greenfield Healthcare Center, a part of the CommuniCare Family of Companies, is seeking an LPN MDS Nurse / Resident Assessment Coordinator to contribute to our operations team. As a key member of our management team, this role plays a crucial part in ensuring the accuracy and timeliness of resident assessments and RAI process coordination.

Responsibilities:

  • The LPN MDS Nurse RAC reports directly to the Executive Director and is responsible for completing mds assessments accurately and on time while coordinating the RAI process.
  • This role provides oversight to ensure Medicare, Medicaid (case mix), and managed care requirements are met to guarantee appropriate reimbursement for each resident.
  • Ensuring compliance with federal and state regulations as well as CommuniCare's guidelines and policies is a vital aspect of this position.
  • This role serves as a key contributor to the facility's success by helping achieve quality outcomes.
