We are currently seeking a compassionate Senior Care Services Coordinator to join our Senior Care Options department. This role is pivotal in assessing eligibility and ensuring that appropriate services are provided to qualifying members.
Key Responsibilities:
- Encourage the independent functioning of members by providing services in the most suitable and least restrictive settings.
- Conduct initial and ongoing evaluations of members' health and functional capabilities, crafting personalized care plans.
- Build strong relationships with consumers to facilitate effective coordination, monitoring, and necessary adjustments to care plans.
- Maintain regular communication with consumers, including home visits, to track progress towards established objectives.
- Ensure meticulous documentation of all interactions and assessments.
Qualifications:
- A genuine passion for aiding others and a commitment to making a positive impact.
- Flexibility to adapt to the evolving needs of consumers and programs.
- A minimum of two years of experience in the human services sector, ideally with the elderly demographic.
- A Bachelor's degree in Social Work or a related discipline is highly preferred; relevant licensure is advantageous.
- Bilingual candidates are encouraged to apply.
