Job Summary

We are seeking a dedicated Licensed Practical Nurse to join our team at Beaver Dam Health Care Center, a skilled nursing home facility that provides high-quality care to our residents.

Key Responsibilities
  • Supervise Certified Nurses Aides under the direction of a Registered Nurse
  • Collect patient care data and monitor patient conditions and family dynamics
  • Contribute to the development of the plan of care and discharge plan for assigned patients
  • Implement patient care utilizing care plans, delegated medical orders, and collaborate with health team members as necessary
  • Assist in evaluating outcomes of patient care planning and teaching, and revise the plan according to changing patient needs
  • Participate in the orientation and education of personnel and students
  • Document the delivery of healthcare and nursing process in accordance with specific unit standards and hospital nursing policy
  • Demonstrate ability to prioritize tasks and complete duties within allotted time
  • Know the rationale for the effect of medications and treatments, and correctly administer same
  • Obtain report from the nurse being relieved from duty and provide report to nurse coming on duty
  • Keep Supervisor or other as appropriate informed about patient status and related matters
  • Perform or supervise complete, timely, and accurate documentation of patient care
  • Perform or supervise administration and documentation of medications, internal nutrition, and treatments per physician's orders, and accurately record all care provided
  • Perform or supervise review of medication and treatment records for completeness, accuracy in transcription of physician orders, and adherence to stop order policy
  • Utilize the nursing process, documentation, communication, and technical skills, collaboration with physicians and other team members to meet performance requirements
  • Perform or supervise the service and documentation of prescribed diets and fluid intake
  • Makes patient rounds to assess physical and emotional status and to initiate nursing intervention
  • Demonstrate willingness to try new tasks, generate new ideas for change, evaluate and recognize priorities, communicate and model organizational values, foster high performance, recognize need for and provide adequate resources
  • Apply process for improvement in daily work, and assist in education of new employees in the team process
Requirements
  • Minimum requirements are a Graduate from an accredited school of licensed vocational nursing or licensed or licensed practical nursing with current state license in good standing
  • Two or more years of acute care or rehabilitation experience as a LPN is preferred
  • Current state license to practice as a LPN. Must maintain current provider CPR certification throughout employment. ACLS certification preferred
