We are seeking a skilled RN to join our Hospice Home Care Team as a Per Diem Case Manager. This is an ideal opportunity for a registered nurse who enjoys working closely with a team to provide comprehensive care to patients and families.
Key Responsibilities- Coordinate with an interdisciplinary team to develop a comprehensive plan of care for patients and families.
- Provide added support to patients and families through effective communication and coordination of care.
- Work closely with a team of healthcare professionals, including Aides, LPNs, medical directors, and NPs, to ensure seamless care delivery.
- A current license to practice as a Registered Nurse in the State of New York.
- Associates Degree in Nursing, BSN preferred.
- 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
- Valid Driver's License and Reliable vehicle.
- The RN must be able to communicate effectively, verbally and written.
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.
- Mileage Reimbursement (not taxed).
- Strong orientation program with training in End of Life Care.
- Flexibility to start your day with your first patient and end with your last patient.
