Job Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Hospice Home Care Team in Columbia/Greene Counties. As a full-time RN, you will provide high-quality care to patients and families in their homes, working closely with an interdisciplinary team to develop comprehensive plans of care.

Responsibilities:

  • Coordinate with the team to develop and implement care plans that meet the unique needs of each patient and family.
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and document findings in the patient's medical record.
  • Collaborate with other healthcare professionals, including physicians, social workers, and chaplains, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and provide emotional support as needed.

Requirements:

  • Current license to practice as a Registered Nurse in the State of New York.
  • Associates Degree in Nursing, BSN preferred.
  • At least one year of experience as an RN, preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and reliable vehicle.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
