Job Summary

We are seeking a compassionate and skilled LPN to join our team as an Individualized Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes and facilities, working under the direction of an RN.

Key Responsibilities
  • Provide individualized care to patients with complex needs, including those who are actively declining.
  • Work collaboratively with the patient's healthcare team to develop and implement personalized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Develop and implement effective care plans that address patients' unique needs and goals.
  • Provide education and support to patients and their families on end-of-life care and related topics.
Requirements
  • Registered LPN with the State of Ohio without any board actions.
  • One year of nursing experience preferred.
  • CPR Certified.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
