Under the direction of a Licensed Professional, you will be responsible for assessing patient needs, developing and implementing care plans, and collaborating with the healthcare team to ensure seamless care delivery.
Key Responsibilities- Assess patient conditions and develop individualized care plans in collaboration with the healthcare team.
- Administer medications and vaccinations safely and effectively, adhering to organizational policies and state regulations.
- Provide education and support to patients, families, and staff members, promoting health literacy and empowerment.
- Coordinate patient care with other healthcare entities, ensuring continuity and coordination of services.
- Lead and mentor LPNs in a clinical setting, promoting professional growth and development.
- Current LPN licensure from the state of employment or LPN applicant.
- BLS certification preferred upon hire and required within 30 days of independent patient care.
- 1 year of clinical patient care experience preferred.
- EPIC Electronic Health Record (EHR) training preferred.
