Key Responsibilities:
- Assessment: Conducts initial and ongoing evaluations to identify care coordination and discharge planning needs based on the patient's clinical condition.
- Planning: Develops a targeted discharge plan tailored for high-risk patients with specific needs.
- Intervention and Evaluation: Works alongside the clinical team and medical providers to implement the discharge plan effectively.
- Quality Improvement: Remains informed about evolving organizational standards, regulatory requirements, and trends in clinical care and discharge planning.
Qualifications:
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) required.
- 3-5 years of relevant experience, with a minimum of 2 years in Case Management, preferably in Surgical or Intensive Care settings.
- Active Maryland Registered Nurse License.
- Intent to obtain Maryland licensure if currently licensed in another state.
- Case Management certification is preferred.
