Job Title: Home Health Licensed Practical Nurse LPN PRN

Job Summary:

Aveanna is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional outcomes.

Responsibilities:

  • Deliver quality patient care in compliance with physician orders under the direction of a Registered Nurse teammate.
  • Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Provide education to clients and their family members based on client-specific needs.
  • Provide care in patients' homes using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes, etc.

Requirements:

  • An active LPN license in the state of application.
  • Valid CPR certification.
  • Preferred: Medicare Skilled Nursing experience, basic understanding of Oasis, 1-year LPN experience in a home health or healthcare setting, Home Care Home Base experience.
