Position Summary
Sovah Health is seeking a dedicated individual to deliver direct patient support, technical assistance, and contribute to care planning through effective communication with registered nurses and other healthcare team members. The role involves actively participating in upholding the quality of care for patients within the designated unit.
Key Responsibilities:
- Perform fundamental patient care tasks as assigned and overseen by a registered nurse, including monitoring and documenting vital signs such as blood pressure, pulse, respiration, and temperature as instructed.
- Assist patients with personal hygiene and daily living activities, including bathing, grooming, eating, and mobility.
- Report any unusual findings or changes in patients' physical, mental, or emotional states to the nursing staff.
- Help maintain cleanliness and organization in patient rooms and communal areas.
- Support patients in using restroom facilities, including bedpans.
- Assist patients with mobility, including the use of walkers and wheelchairs when necessary.
- Provide assistance with feeding as required.
- Monitor and document patients' fluid intake and output as directed.
- Reposition patients as needed to ensure comfort and care.
- Check and maintain patient hygiene with each position change, providing linen changes as necessary.
- Utilize proper patient identification protocols, ensuring two identifiers are used.

Minimum Qualifications:
Education:
A high school diploma or equivalent is required. Candidates must be enrolled in a professional nursing program and have completed their first semester (Fundamentals of Nursing), or be a graduate of a nursing program who has not yet passed the NCLEX. Employment as a Patient Care Assistant is permissible for 180 days post-graduation from a professional nursing school.
Required Skills:
Critical thinking, sound judgment, and the ability to work independently are essential. Candidates must thrive in a high-pressure environment and respond appropriately to various situations.
Flexibility in scheduling is required, with the ability to work various shifts, including weekends and holidays, to ensure continuous patient care and nursing coverage.
Certifications:
Basic Life Support (BLS) certification from the American Heart Association is mandatory.
