Job Title:

Patient Care Coordinator - Behavioral Health

Job Summary:

Boston Medical Center is seeking a skilled Patient Care Coordinator to join our Behavioral Health team. The ideal candidate will have experience in inpatient psychiatric settings and a strong understanding of mental health treatment plans.

Responsibilities:
  • Provide evidence-based evaluation, diagnosis, and treatment of patients, including prescribing medications and/or other psychotherapeutic interventions.
  • Develop treatment plans, measure progress against the plans, and make adjustments as needed.
  • Collaborate with a multidisciplinary team to assess and adjust treatment plans.
  • Complete all necessary clinical documentation in an accurate and timely manner.
  • Participate in departmental and organizational quality improvement programs.
Requirements:
  • Master's Degree from a Nurse Practitioner education program (with specialization in mental health) accredited by a Massachusetts Board of Registration in Nursing-recognized national accreditation body.
  • At least 2 years of experience as a Nurse Practitioner in an inpatient, adult psychiatric setting.
  • Current licensure as a Nurse Practitioner in the Commonwealth of Massachusetts.
  • Current valid DEA certificate and MA Controlled Substances certificate or eligibility.
  • Buprenorphine waiver required or willingness to acquire waiver.
  • Current advanced practice certification by Massachusetts Board of Registration in Nursing- approved nationally recognized certifying body.
  • No Medicare or Medicaid sanctions.
