Position Overview
The Licensed Practical Nurse (LPN) plays a crucial role in delivering and documenting skilled nursing services under the guidance of a Registered Nurse. This position adheres to established care plans and physician directives for each patient while maintaining confidentiality and professional boundaries.

Key Responsibilities
  • Deliver and coordinate high-quality patient care in accordance with physician orders.
  • Continuously monitor and evaluate patient conditions, reporting any changes to the supervising nurse or physician.
  • Document all nursing activities, assessments, and interventions promptly and accurately.
  • Participate in the development and updating of individualized nursing care plans.
  • Implement appropriate nursing actions based on patient assessments to achieve desired health outcomes.
  • Recognize and respond to changes in patient needs, providing interventions to mitigate risks.
  • Comply with all policies, regulations, and the Nurse Practice Act to uphold personal and professional accountability.
  • Utilize infection control measures to safeguard both patients and staff in accordance with OSHA standards.
  • Educate patients and their families about disease processes, self-care techniques, and preventive measures.
  • Engage in ongoing professional development through educational programs and skill evaluations.

Qualifications
  • Graduation from an accredited nursing program.
  • Possession of a current, unrestricted state license as a Licensed Practical Nurse.
  • Current CPR certification.
  • Proficiency in clinical assessments and adherence to nursing policies and procedures.

Preferred Experience
  • At least six months of recent nursing experience in a clinical setting.
  • Experience in home health care.

Physical Requirements
  • Ability to communicate effectively in English, both verbally and in writing.
  • Capability to lift up to 50 pounds and perform physical tasks related to patient care.
  • Ability to navigate various environments and respond to emergency situations appropriately.

Work Environment
