Job Summary

Brighton Hospice is seeking a skilled Care Coordinator to join our team. As a key member of our clinical support team, you will play a vital role in ensuring the smooth admission process for our patients.

Key Responsibilities:

  • Collaborate with our Office Manager and Director of Clinical Services to ensure seamless patient admissions
  • Support our site Monday through Friday, on-site at our Newton office, to assess appropriate levels of care and patient eligibility for hospice admission
  • Establish and maintain positive working relationships with current and potential referral sources
  • Assign referrals to each location and communicate schedules to the Director of Clinical Services promptly for admissions

Requirements:

  • Associate degree or equivalent knowledge and experience, preferably in the hospice/healthcare field
  • Excellent verbal and written communication skills, including telephone etiquette and negotiation skills
  • Proficient in Microsoft Office and maintain a positive and professional attitude
