Job Summary:

The Case Management Specialist plays a vital role in promoting optimal clinical and resource outcomes for patients. This position requires excellent customer service skills and a commitment to delivering Extraordinary Care for Every Generation.

Key Responsibilities:

• Facilitate collaborative patient care management across the continuum, intervening as necessary to ensure timely and efficient care delivery.
• Coordinate patient transfers to the most appropriate level of care, adhering to CMS Discharge Planning regulations.
• Coordinate care for patients during their hospital stay and effect timely transitions to sub-acute settings that meet their needs.
• Communicate effectively with physicians and other healthcare team members to ensure seamless care delivery.
• Participate in process improvement methodologies to evaluate outcomes of care and interventions.
• Demonstrate knowledge and skills necessary to provide care appropriate to the patient population served.

Requirements:

• Must be able to demonstrate knowledge of the principles of growth and development as it relates to different life cycles.
• Must be able to serve as a patient and family advocate, contributing to organizational targets for length of stay reductions.
• Must be able to build relationships within the organization and serve as a resource to the healthcare team related to discharge planning.
