We are seeking a skilled Wound Care Nurse Practitioner to join our team at CareMed Health Solutions. As a Wound Care Nurse Practitioner, you will play a critical role in providing high-quality wound care services to patients residing in long-term care facilities.

Responsibilities:
  • Provide comprehensive wound care services, including assessment, diagnosis, and treatment of chronic, debilitating, stalled, or infected wounds.
  • Prescribe medications, including controlled substances, as needed, and order diagnostic testing to support patient care.
  • Collaborate with the interdisciplinary care team to develop and implement individualized care plans that address the unique needs of each patient.
  • Communicate effectively with nursing home staff to ensure seamless care coordination and timely updates on patient status.
  • Stay up-to-date with the latest wound care best practices and advanced wound care products to provide evidence-based care.
Requirements:
  • Current, unrestricted NP license in the state of Kentucky.
  • Board Certification in a relevant specialty.
  • DEA licensure (optional).
  • Experience working in a long-term care setting and a strong background in adult, family, or geriatric specialties.
  • Ability to work with an EMR and collaborate with an interdisciplinary care team.
