About the Role

We are seeking a highly skilled and experienced Clinical Resource RN to join our team at Pennant Service Center in Hartford, Connecticut. As a Clinical Resource RN, you will play a critical role in supporting local clinical leaders at agencies located throughout the northeast.

Key Responsibilities
  • Design, monitor, implement, and evaluate processes and programs to meet regulatory and accreditation requirements and achieve the highest standard of care.
  • Provide leadership and guidance in the establishment of programs, policies, and practices that ensure exceptional clinical care in compliance with State and Federal Regulatory requirements.
  • Educate and assist local clinical leaders to be survey-ready and work closely with compliance resources to ensure appropriate and effective survey preparedness.
  • Assist local leadership in developing and implementing processes to monitor and measure clinical, education, and performance improvement activities.
  • Provide support regarding the direction and coordination of educational programs, orientation, new hire competency systems, annual skills competency, and ongoing in-service educational program development.
  • Monitor the organization's Outcome-Based Quality Initiative (OBQI) outcomes and provide feedback and best practice education to staff to improve outcomes.
  • Compile, trend, and report quality data in areas such as patient care, safety, risk management, infection control, outcomes, and customer satisfaction.
Requirements
  • Current Licensed Registered Nurse
  • CPR Certified
  • A minimum of five years' Home Health and/or Hospice experience preferred
  • Ability to work with all levels of management on all aspects of clinical operations
  • Verbal and written communication skills necessary to explain complex and/or confidential information
  • Time management and organizational skills necessary to manage multiple projects, prioritize workload, plan for agency leaders to meet deadlines and goals, and work independently in responding to day-to-day functional needs
