Key Responsibilities:- Develop and supervise staff, including RNs, SWs, and non-clinical support personnel.
- Evaluate and maintain case management skills and outcomes.
- Manage all policies and procedures related to case management.
- Ensure adherence to key model components that drive outcomes.
- Strong analytical abilities to strategically plan, consensus build, and problem solve.
- Ability to communicate complex information in a professional and courteous manner.
- Administrative skills to supervise and develop staff, plan, organize, direct, and have proficiency in data analytics.
- Strong understanding of clinical and financial outcomes.
- Strong interpersonal skills to interact effectively with a wide range of personalities, ages, and diverse cultural backgrounds.
- Minimum 5+ years of progressive management experience with demonstrated goal-oriented outcomes.
- Demonstrated leadership and change management experience.
- Case Management experience preferred.
