Job Summary

We are seeking a highly skilled Social Work Case Manager to join our team at Novant Health. As a leader of the complex discharge planning process, you will collaborate with physicians and other healthcare professionals to assess, plan, implement, and evaluate proactive, complex, and safe discharge plans.

Key Responsibilities
  • Collaborate with RN Case Managers for guidance on medically complex cases
  • Facilitate coordination of care and serve as a resource for discharge planning and care coordination for psychosocially complex cases
  • Maintain current knowledge of available community resources and post-acute care options
  • Develop and implement effective discharge plans that meet the needs of patients and families
  • Provide education and support to patients and families on discharge planning and care coordination
Requirements
  • Master's Degree in Social Work from a school accredited by the Council on Social Work Education
  • Two or more years of professional social work experience or two or more years of healthcare experience, preferred
  • Case Management or related experience, preferred
  • NC state-approved curricula for restrictive interventions (facility-specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required
