Overview:
We are seeking a skilled Case Manager to join our team at Novant Health. As a Case Manager, you will play a critical role in facilitating complex discharge planning and collaborating with healthcare professionals to ensure the best possible outcomes for our patients.
Responsibilities:
- Assess, plan, implement, and evaluate proactive, complex, and safe discharge plans in collaboration with physicians and other healthcare team members.
- Collaborate with RN/MSW Case Manager for guidance on medically and/or psychosocially complex cases.
- Maintain current knowledge of available community resources and post-acute care options.
- Provide exceptional patient care and advocate for patients' needs.
Requirements:
- 4-year Bachelor's Degree in Social Work from a school accredited by the Council on Social Work Education.
- Two or more years of professional social work experience or two or more years of healthcare experience, preferred.
- NC state-approved curricula for restrictive interventions within 3 months of hire for Emergency Departments and Behavioral Health units.
- Weekend and holiday coverage.
