Clinical Transition Specialist RN ED

Job Summary:
The Clinical Transition Specialist RN ED will conduct comprehensive patient/family assessments and admission reviews to ensure appropriateness of care settings and timely implementation of plans of care. This role will work directly with the Emergency Room MD/RN/Patient/Families to establish and implement plans of care, facilitating positive patient outcomes. The nurse will also work with outside resources to ensure safe discharge plans.
Key Responsibilities:
- Meet with patients and families to discuss plans of care and use the nursing process to assist patients and families through the emergency room experience.
- Identify high-risk patients and their needs, and provide recommendations for standard of care treatments that can be safely rendered at home.
- Collaborate with physicians, primary nurses, care team leaders, staff nurses, social workers, and other care disciplines to develop, implement, and evaluate plans of care.
- Demonstrate complex clinical decision-making skills and ability to advocate for patients and families within the healthcare system.
- Collaborate with medical staff to recommend and arrange post-acute care, including outpatient care, home health, or other community agencies as indicated.
- Encourage patients and families to consider their goals of care and work collaboratively with providers to determine appropriateness of palliative or hospice care.
- Provide early identification of high-risk patients and their needs, and work with outpatient care coordinators and/or primary care physicians to develop plans of care to avoid ED visits and hospitalizations.
Requirements:
- Associate's degree or diploma in Nursing
- Registered Professional Nurse (RN) License in Illinois upon hire
- Accredited Case Manager Certification within 3 years
- Three (3) years clinical experience
Skills and Knowledge:
- Case management experience preferred
- Ability to critically evaluate and analyze physical and psychosocial assessment data
- Ability to lead, coordinate, and facilitate patient progression throughout the continuum
