Clinical Transition Specialist RN Job Description

At Carle Foundation Hospital, we are seeking a skilled Clinical Transition Specialist RN to join our team. As a Clinical Transition Specialist RN, you will be responsible for overseeing the coordination and management of patient care during acute illness, ensuring a smooth transition from hospital to home or other care settings.

Key Responsibilities:

  • Conduct proactive initial assessments of patient needs and develop individualized plans of care.
  • Coordinate with healthcare teams to ensure timely and effective transitions of care.
  • Monitor patient progress and make adjustments to plans as needed.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.

Requirements:

  • Associate's Degree in Nursing and Bachelor's Degree in Nursing required within five years of start date.
  • Registered Professional Nurse (RN) License in Illinois.
  • Accredited Case Manager Certification (highly encouraged within three years).
  • Three years of related experience in case management or a related field.
